Gentle Dentistry of Poway ("Practice," "we," "us") is required by law to maintain the privacy of your protected health information (PHI) and to provide you with this Notice of our legal duties and privacy practices with respect to PHI.
We are required to abide by the terms of this Notice currently in effect. We reserve the right to change the terms of this Notice and to make the new Notice provisions effective for all PHI that we maintain. We will post a revised Notice in our office and on our website.

Treatment: We may use or disclose your PHI to provide, coordinate, or manage your dental care and any related services, including sharing information with other healthcare providers involved in your treatment.

Healthcare Operations: We may use or disclose your PHI in connection with our healthcare operations, including quality assessment, training, accreditation, licensing, and other business management activities.

We do not collect PHI through our website contact forms beyond what you voluntarily provide. Appointment request submissions are transmitted securely and used solely to respond to your inquiry.

5. California Privacy Rights (CCPA/CPRA)
